Medical Record and Immunization Policy


CGU seeks to provide a safe and healthy environment to all on campus and encourages students to take an active role in their own health. CGU students are required to verify immunization to the following and submit the Medical History Report, request an exemption or submit an affidavit prior to arriving on campus. Beginning fall 2022, CGU will require the following screenings:

  • Tuberculosis Screening
  • MMR (measles/mumps/rubella)
  • Td or Tdap (tetanus/diphtheria/pertussis)
  • Varicella (chickenpox)
  • COVID-19 – CGU students are required to be vaccinated against COVID-19 prior to coming to campus. Refer to current guidance online and upload your vaccination card to Student Health Services.

  • In addition, the following vaccines are recommended to prevent illness: influenza, meningococcal, human papillomavirus (HPV), hepatitis A, hepatitis B, and polio.

To protect your privacy, all medical forms must be sent to Student Health Services directly at shsrecords@cuc.claremont.edu and not to any other office at CGU.  More information on the importance of immunization is available at Student Health Services.
