Medical Record and Immunization Policy

CGU seeks to provide a safe and healthy environment to all on campus and encourages students to take an active role in their own health. Admitted and continuing students are strongly urged to verify immunization to the following and submit the Medical History Report prior to arriving on campus. CGU’s current policy requires all students submit a tuberculosis screening. Beginning fall 2022, CGU will recommend the following vaccinations and immunizations:

• MMR (measles/mumps/rubella)
• Td or Tdap (tetanus/diphtheria/pertussis)
• Varicella (chickenpox)
• In addition, the following vaccines are recommended to prevent illness: influenza, meningococcal, human papillomavirus (HPV), hepatitis A, hepatitis B, polio and COVID 19

To protect your privacy, all medical forms must be sent to Student Health Services directly at shsrecords@cuc.claremont.edu and not to any other office at CGU.  More information on the importance of immunization is available at Student Health Services.
